About The Position

The Behavioral Health Clinician (BHC) works in a consultative and collaborative role as an integral member of the primary care team. They provide behavioral health interventions to primary care patients with the goal of improving overall health of patients. The BHC helps address a broad spectrum of behavioral health needs in the primary care patient population. Their goal is to aid in early identification and provide problem focused interventions that promote wellness and relieve symptoms. Behavioral issues that are addressed may be related to physical or mental health conditions. They provide assessment, management and triage of psychological and social problems in the clinic patient population.

Requirements

  • Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington.
  • Minimum of two years of clinical experience or healthcare related field.
  • Current valid Oregon and/or Washington License as LCSW, LICSW, LPC, LMHC or LMFT.

Nice To Haves

  • Modalities including ACT, CBT, CBT-I, and psychodynamic psychotherapy are all applicable.
  • Flexibility and willingness to work at the fast pace of primary care.

Responsibilities

  • Provide assessment, management and triage of psychological and social problems in the clinic patient population.
  • Provide behavioral health interventions to primary care patients with the goal of improving overall health of patients.
  • Aid in early identification and provide problem focused interventions that promote wellness and relieve symptoms.
  • Meet with patients individually for an assessment and then providing recommendations to the primary care team to enhance the patient's care plan.
  • Work with patients for several subsequent sessions to provide on-going skills training and coaching to improve health and functioning.
  • Support helping patients, who require services not provided in the LMG clinic, to make a smooth transition to care with community partners.
  • Diagnose common mental health problems.
  • Conduct solution focused interventions.
  • Recognize and advocate for patients regarding behavioral health strategies/interventions.
  • Promote health behavior change through evaluation, brief intervention and education.
  • Work with multidisciplinary team to help support the patient’s health.
  • Coordinate with LMG providers and community partners to refer for appropriate mental health therapy.
  • Assess emotional, cognitive and behavioral disorders in child, adolescent, adult and/or geriatric patients (depending upon population served by specific position).
  • Assess cognitive and psychological problems associated with, and/or exacerbating medical conditions.
  • Work with an interdisciplinary team.
  • Provide case conceptualization and consultation.
  • Work at the fast pace of primary care.
