About The Position

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Intake Specialist to join their dynamic Flagstaff team. This specialty position works with adult individuals who have been released and are transitioning from incarceration. The Intake Specialist is part of a multi-disciplinary team with the Pathways Program and the County Jail System, aiming to successfully transition individuals from jail services to community stability. This role involves interfacing with correctional and legal system staff, providing direct client contact and service, case management, community outreach, service planning and coordination, crisis and behavioral health interventions, referrals, and follow-up. The position partners with those served to ensure success and supports team processes.

Requirements

  • High School Diploma or GED plus some college coursework in Behavioral Health or related fields, plus one year of directly related job experience, or the equivalent combination of experience and education.
  • Minimum requirement consists of a 22-year age.
  • Required to maintain a valid Arizona driver’s license and pass motor vehicle screening for insurance purposes.
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.

Responsibilities

  • Completes intake/enrollment for individuals released from incarceration.
  • Facilitates connection to outpatient services and behavioral health/psychiatric medication appointments, including MAT induction and continuation.
  • May provide crisis triage support as needed.
  • Facilitates linkages to care, including warm handoffs to community partners, for behavioral health care coordination.
  • Works cooperatively as part of a multi-disciplinary team with Pathways staff.
  • Develops working rapport with the team, persons served, and the legal system to ensure smooth transitions.
  • Responsible for case management and carries an independent caseload.
  • Follows up on all referrals in a timely manner.
  • Provides screenings and ongoing assessments.
  • Provides crisis intervention, de-escalation, and Behavioral Health interventions.
  • Acts as a responsible guest of the jail, assessing client situations, recommending services, coordinating care, and developing rapport.
  • Provides outreach work based on team and individual needs.
  • Works to reduce recidivism and meet program performance outcome goals.
  • May work with persons to reestablish benefits.
  • Reviews release expectations with clients and follows up as needed.
  • Plans, coordinates, and assists clients with immediate and basic needs for release.
  • Makes referrals to appropriate services in cooperation with clinical teams and monitors client participation and compliance.
  • Works to provide or facilitate prescribed treatment.
  • Develops and implements a tracking system and follow-up process for referrals.
  • Documents activities for tracking and reporting according to documentation standards.
  • Responsible for contractual obligations and fulfillment.
  • May be required to work flexible hours, including weekends, as needed.
  • Required to carry a company smart phone.
  • Required to drive a personal or company vehicle and maintain appropriate documentation.
