Intake/Scheduling Coordinator (Level 2) - San Francisco
About The Position
For nearly 20 years, Health Link Home Health & Hospice has been a leader in Bay Area healthcare. We are looking for a high-energy, detail-oriented Intake & Scheduling Coordinator to join our San Francisco team. As the "Air Traffic Controller" of our office, you ensure that patients transitioning from hospitals to their homes receive care without delay. This is a fast-paced role that requires someone who can juggle logistics while maintaining a compassionate touch. To provide world-class care, our office remains responsive seven days a week. This is a Full-Time position. Monday through Friday regular shift. Requirement: Participation in a Rotating Weekend Coverage schedule is essential to this role. Requirements
- Experience: Minimum 1 year in healthcare intake/scheduling (Home Health or Hospice experience is a major plus!).
- Communication: Ability to speak professionally with doctors, families, and clinicians.
- Adaptability: You thrive in a "moving parts" environment and can pivot priorities quickly.
- Tech-Savvy: Comfortable navigating healthcare CRMs and medical terminology.

Responsibilities
- Manage Referrals: Process incoming patient referrals from hospitals and providers with speed and accuracy.
- Coordinate Care: Work directly with patients and clinical teams to assign and schedule home visits.
- Problem Solve: Proactively troubleshoot scheduling conflicts to ensure continuity of care.
- Data Management: Maintain flawless records in our software systems.
