About The Position

The Intake Patient Coordinator is crucial in facilitating the admission process for patients entering hospice and palliative care services. The Intake Patient Coordinator performs a variety of administrative support duties associated with the hospice intake and/or referral process following internal processes and procedures. Additionally, the Intake Patient Coordinator acts as a knowledgeable resource, offering essential information to families considering hospice and palliative care options.

Requirements

  • High School Diploma or Equivalent
  • Must be able to pass a pre-employment drug screen which includes cannabis testing in Washington State.
  • Ability to report to the assigned work location(s) as scheduled and maintain consistent, reliable attendance.
  • Two (2) years of related work experience in a medical office, healthcare facility, hospice, or palliative care setting.
  • Two (2) years of experience in medical terminology and abbreviations.
  • Strong communication and interpersonal skills, with the ability to interact compassionately with patients and families.
  • Excellent organizational and time management skills, with attention to detail.
  • Ability to work effectively in a fast-paced environment.
  • Proficiency in computer applications, Microsoft Office, and electronic medical records (EHR) systems.
  • Ability to write concisely, and legibly and follow written and verbal instructions.
  • Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
  • Maintain reliable personal transportation.
  • Maintain a valid driver’s license and vehicle insurance per state requirements.
  • Satisfy state and agency health requirements by providing proof of immunization, declinations, or approved accommodations as applicable.
  • Pass criminal background and drug screenings.
  • Meet the physical and environmental requirements of the position, with or without reasonable accommodation.
  • Complete agency and departmental orientation and required training programs.
  • Complete ongoing education, policy reviews, and training be designated deadlines.
  • Maintain current licensure(s) and certifications(s) required for the role.
  • Read, write, speak, and understand English fluently.
  • Be available to work as assigned or scheduled.

Responsibilities

  • Facilitate the admission process for patients entering hospice and palliative care services.
  • Perform administrative support duties associated with the hospice intake and/or referral process following internal processes and procedures.
  • Act as a knowledgeable resource, offering essential information to families considering hospice and palliative care options.
  • Participate in education and outreach activities to promote agency programs, including hospice care, advanced planning, and bereavement services.
  • Maintain knowledge of and comply with agency policies, procedures, and practices.
  • Protect patient and agency information per HIPAA guidelines. Only access information on a need-to-know basis.
  • Complete agency and departmental orientation and required training programs.
  • Complete ongoing education, policy reviews, and training by designated deadlines.
  • Maintain current licensure(s) and certifications(s) required for the role.
  • Read, write, speak, and understand English fluently.
  • Be available to work as assigned or scheduled.
