Intake Nurse
About The Position
Facilitate admissions for incoming Skilled Nursing patients. This role does not include direct patient care. Responsibilities include verifying and entering physician orders, identifying the level of care needed, receiving and giving verbal orders accurately, and reviewing clinical documentation to determine Skilled Nursing placement. The position requires exceptional interpersonal and customer service skills, the ability to communicate clearly with medical providers, and close collaboration with referring hospitals, physicians, skilled nursing facilities, patients, and families. The role also involves managing after-hours and weekend referrals on a rotating on-call basis. The schedule is Monday – Friday, day shift, with rotating weekends and on-call duties approximately every 6 weeks. This position complies with the CMS Mandate for COVID 19 Vaccinations.
Requirements
- Current license as a licensed LPN/RN from the Utah State Board of Nursing
- 1-year Skilled nursing facility experience
- Good verbal and written communication skills
- Basic computer knowledge
- Ability to effectively problem solve
- Provide excellent customer service
- Knowledge of Medicare, Medicaid and 10A criteria
