About The Position
The Intake Nurse is a qualified LPN responsible for receiving, processing, and managing all agency referrals. The role assists with hospital-to-home discharge processes and serves as a liaison with social service and case management departments within hospitals and physician offices.
Requirements
- Graduate of an accredited RN/LPN program.
- Minimum of 2 years of nursing experience.
- Current Indiana RN/LPN licensure.
- Current CPR certification.
Nice To Haves
- One year of home health experience preferred.
- Knowledge of EMR (Epic), Microsoft, Excel, Word, and Outlook preferred.
