We are looking for an experienced, talented, and compassionate Intake RN or LPN to work 40 hours/week. In this position you would work Monday - Friday, this role is hybrid with 4 days onsite in our office located in Norwell, MA and 1 day remote.

Requirements

• 3+ years of nursing experience
• Current Massachusetts License (in good standing).
• Medicare/Managed Care experience required.
• Familiar with medical data base programs (i.e. Health Wyse, etc.).

Nice To Haves

• 1+ year in direct home health care preferred.
• Strong clinical assessment skills and the ability to prioritize and triage patients.
• Exceptional communication skills and the ability to work within a team structure.
• Excellent organizational skills and time management skills.

Responsibilities

• Overseeing the appropriateness of referrals to the Agency’s Intake Department and ensuring that the processing of referrals for home care services is completed on a timely basis.
• Working with Clinical Managers in assisting to coordinate the Agency’s daily workload and appropriateness of patients for agency services.
• Building relationships, educating and supporting referral sources.
• Overseeing process new referrals with Intake CTAs.
