About The Position

Responsible for all aspects of the referral and intake process to ensure individuals interested in Life Unlimited services are placed in settings of their choice in a timely manner. The Intake Manager oversees all operations of the intake process and the success of new individuals successfully transitioning into Life Unlimited services. In addition, the Intake Manager will support the Community Living Program Managers to facilitate internal moves.

Requirements

  • Bachelor’s degree in a related field desired or 8 years of related experience
  • Five (5) – ten (10) years of experience working with individuals with disabilities.
  • Must obtain and maintain training and certification in all required areas such as Abuse/Neglect, CPR, First Aid, Level I Medication Administration, Positive Behavior Supports, TOOLS, Crisis Intervention.
  • Must obtain a Class E driver’s license during new hire orientation
  • Must carry minimum coverage applicable by law for auto insurance
  • Must Receive Seasonal Flu Vaccination
  • Ability to effectively and efficiently use computers or mobile devices to input, access, modify, or output information. This includes the ability to quickly access, store and retrieve data.
  • Good communication skills both orally and in written form.
  • Ability to use computer and other electronic equipment.
  • Must maintain a flexible schedule to accommodate the needs of individuals, families, and/or guardians.
  • Ability to plan and prioritize to meet deadlines.
  • Must be willing to perform duties that are assigned outside of this job description.
  • Must possess self-motivation, initiative and independence to work without direct supervision.
  • Understands that transition into our agency is sometimes difficult for individuals and their families/guardian and works to help everyone involved feel comfortable and reassured.

Responsibilities

  • Ensure residential vacancies are less than 5% and strive for the constant growth and expansion of new individuals in the Community Living Department.
  • Serve as the supporter and advocate for individuals and families/guardians thinking about services provided by LU.
  • Organize all aspects of initial emergency residential placements.
  • Oversee residential placement into temporary transition housing and subsequent placement into a permanent home.
  • Attend weekly/monthly roommate match meetings and any other similar meetings.
  • Actively work to find new individuals, coordinate meet and greets with pertinent parties, and ensure good roommate matches with potential housemates.
  • Complete the initial intake packet for referrals received and maintain constant contact with the individual, family/guardian and Service Coordinator until a provider is chosen.
  • Initiate and organize the first meeting and tour with the individual, family/guardian, along with Program Manager and Director, health team, etc. and monitor progress through first 60 days of placement.
  • Handle all aspects of the transition to the agency until move in date is determined and all paperwork has been approved and in CIMOR.
  • Participate in Pre and Post Transition Meetings with Service Coordination and Regional Office.
  • Work closely with Unlimited Property Management (UPM), Maintenance Team, and Program Managers to coordinate residential placements.
  • Ensure communication of timelines between newly identified individual, their families and Service Coordinator, UPM, Maintenance, and Program Managers.
  • Determine housing/modification needs and obtain approval and timeline from UPM for completion.
  • Coordinate movers, utility set up, safety/evacuation plan completion, and new home walk thru with UPM, Maintenance and Service Coordination.
  • Be present the day of move to ensure smooth transition for new/current individual, family members, and the residential staff.
  • Ensure post-placement needs identified are addressed within the first 30 days of placement.
  • Act as contact person to coordinate the initial MAAS Assessment, participate in MAAS Assessments as 'informant' and participate in 90 days post move.
  • Coordinate and assist Program Managers with inner agency moves to ensure consistent information is shared throughout the agency.
  • Maintain required documentation and ensure confidentiality.
  • Maintain familiarity with and implement all accreditation standards as well as all other requirements mandated by the contracting agency.
  • Maintain confidentiality according to HIPAA practices.
  • Maintain Intake database and ensure it is updated and current for all LU programs.
  • Create Therap Account for newly enrolled individuals to ensure all requirements are met which includes but is not limited to individuals' demographics, individual's emergency form, Contact List, and ISP programs, etc. are complete.
  • Work with the program leadership to ensure all required documentation has been received and uploaded into Therap Documentation Storage.
  • Ensure all required documents are obtained 30 days prior to individual participating in Life Unlimited Inc. services.
  • Maintain a record system of potential/discharged individuals.
  • Responsible for timely and ongoing communication and satisfaction.
  • Responsible for timely and ongoing communication with Life Unlimited Inc Teams members.
  • Interact in a professional manner with agency staff, personnel, and internal/external stakeholders to promote high quality services and support.
  • Work closely with support coordinators regarding referrals while maintaining a professional and open line of communication with support coordinators.
