About The Position

Central Florida Kidney Centers, Inc. has served the Central Florida community for over 50 years as a mission-driven, not-for-profit dialysis provider. We are dedicated to delivering high-quality, patient-centered kidney care while supporting the emotional, social, and clinical needs of every individual we serve. We are seeking a licensed, experienced social worker to lead our Intake Department as a Dialysis Intake Manager. This role is ideal for a professional who combines strong clinical assessment skills with leadership experience and a deep understanding of care coordination. You will oversee the intake process from referral to admission, ensuring patients transition into care smoothly, efficiently, and with the support they need. This role plays a critical part in shaping the patient experience at the very first point of contact.

Requirements

  • Licensed Social Worker (LCSW or LMSW preferred)
  • Bachelor's degree required
  • 5+ years of healthcare experience, with emphasis in social work, case management, or care coordination
  • Leadership or supervisory experience required
  • Strong knowledge of insurance, discharge planning, and patient resource coordination
  • Excellent interpersonal skills with the ability to support patients and families
  • Experience working with diverse populations and complex social needs
  • Proficiency in EHR systems and healthcare operations

Nice To Haves

  • Master of Social Work (MSW) strongly preferred
  • Dialysis or chronic disease management experience highly preferred

Responsibilities

  • Lead and manage daily intake operations across clinics, ensuring timely and efficient patient admissions
  • Conduct or oversee psychosocial and care needs assessments for incoming patients
  • Guide patients and families through the intake process, addressing barriers such as insurance, transportation, and social support
  • Collaborate closely with physicians, nurses, social workers, and administrative teams to coordinate care plans
  • Ensure compliance with CMS, state regulations, and organizational standards
  • Supervise, coach, and develop intake team members, fostering a compassionate and accountable culture
  • Track intake metrics, referral trends, and conversion rates to improve processes and patient access
  • Partner with hospital discharge planners and community providers to strengthen referral relationships
