About The Position
Traditional Home Health Care, a leader in the industry with a CMS 4.5 Star rating in Northeast Pennsylvania, is seeking an Intake LPN. This full-time position involves handling all phone inquiries and taking complete referrals for Home Health and Hospice services. The Intake Nurse plays a crucial role in building community relationships with referral sources. Excellent customer service and communication skills are essential for this role. The primary functions include processing referrals and verifying insurance.
Requirements
- Current Pennsylvania LPN license
- Excellent customer service and communication skills

Responsibilities
- Answers phones professionally
- Takes all necessary information on referrals to insure all demographics, diagnoses, and medical orders are complete
- Makes initial determination if patient can be accepted based on insurance type, patient location, and ability to staff
- Collaborates with clinical team to insure all accepted referrals are processed for admission and scheduling
