This role is crucial for building community relationships with referral sources and involves processing referrals and verifying insurances. Requirements
- Current Pennsylvania LPN license

Responsibilities
- Field all phone inquiries and take complete referrals for Home Health and Hospice services.
- Process referrals and verify insurances.
- Answer phones professionally.
- Take all necessary information on referrals to ensure all demographics, diagnoses, and medical orders are complete.
- Make initial determination if a patient can be accepted based on insurance type, patient location, and ability to staff.
- Collaborate with the clinical team to ensure all accepted referrals are processed for admission and scheduling.
