About The Position

This candidate must reside in New Hampshire. This is a per diem, hybrid position with work performed remotely (in NH) and/or in the New London office. The ideal candidate is self-motivated, tech-savvy and an excellent communicator. We are looking for a team player who is willing to jump in and help, is flexible, able to prioritize and stay organized. A detail oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Intake Department as a per diem teammate. This person will assist with both Homecare and Hospice referrals.

Requirements

Active NH Licensed Practical Nurse (LPN) or Active NH Registered Nurse (RN) License
Must reside in New Hampshire
2 years of Health Care experience
Knowledge of medical terminology, familiarity with a medical office setting, and/or triage
Knowledge of Home Health/Hospice
Must be comfortable working in a remote environment
Proof of Flu vaccination
Reliable transportation
Valid driver's license
Automobile insurance
Physical exam within the last year
Two-step TB test

Responsibilities

Accurately enter patient referral information into the electronic medical record system
Accurately enter all incoming patient care orders and notify appropriate team members as necessary
Navigate multiple electronic referral software portals
Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships
Triage all incoming phone calls
Communicate effectively with patients and patient care givers
