About The Position

While assisting the Intake District Manager with the carrying out the vision of the agency, this position is responsible for assisting with the oversight of intakes conducted for all four counties within the San Andreas Regional Center catchment area. This is a key lead position which has an essential role in supporting the intake district manager with the referral process and providing qualitative professional services to consumers, their families, and the community.

Requirements

  • A Bachelor’s Degree in social work, rehabilitation counseling or a related field from an accredited college.
  • A minimum of five years of service coordination experience.
  • Knowledge of the agency; knowledge of the laws, rules and regulations governing the operation of the agency; knowledge of the principles, objectives and techniques of social work, individual and group behavior, human growth, development of family relationships; medical-psychosocial, socioeconomic factors affecting individuals and families.
  • Knowledge of supervisory principles and how to delegate work and assignments
  • Computer Skills
  • Strong/effective written and verbal communication.
  • Use of an automobile, possession of a valid driver’s license and automobile liability insurance for the minimum amount prescribed by law or the ability to provide for independent transportation.

Responsibilities

  • Develops and facilitates intake inquiry orientations/presentations for applicants, employees, and the general public
  • Develops performance objectives in collaboration with district manager so that continuous improvement can occur
  • Sets standards of accountability for staff in producing required intake case management documentation
  • Responsible for tracking and reporting of monthly intake statistical data
  • Provides comprehensive training to new staff and ongoing training after the initial training period is over
  • Participates in and/or leads collaborative practice with various community agencies
  • Prepares and provides professional reports which relate to intake process, including intake inquiry tracking
  • Is an expert on generic resources and assists with developing this knowledge among the service coordinators
  • Gathers data and produces initial and/or ongoing assessments
  • Creates/assigns inquiry numbers for all applicants for all counties
  • Identifies and collects records needed for an intake assessment
  • Presents assessment data to Interdisciplinary Team (IDT) for intake purposes, including appropriateness of referral and eligibility determination
  • Handle intake referrals made via various modes of communication including but not limited to phone calls, walk-ins, email inquiries, fax, etc.
  • Expertise in regional center eligibility criteria, services, and due process
  • Properly assess and determine the next steps for referrals
  • Knows all resources that serve our consumers who reside in various districts
  • Intervenes effectively in crisis situations in order to eventually effect stability
  • Knows Title 17 and Title 22 licensing and DDS requirements
  • Assures that all consumers and families are informed verbally and in writing of their right to appeal every time a decision is made to change, alter, or deny a service, including eligibility for an intake assessment
  • Travel to all four counties in catchment area as needed
  • Other duties as assigned
