Our family of services comprises three distinct mental health treatment programs, including a specialized program for US Veterans. With over 15 unique locations, we manage and support 500+ dedicated employees serving over 300 residential clients daily located across five states: California, Idaho, Maine, New Hampshire, and Texas. The Intake & Discharge Coordinator oversees and facilitates all aspects of the clinical intake and discharge process. This role is highly client-facing and requires strong organizational skills, clear communication, and close collaboration with clinical, admissions, and program teams to ensure a smooth and supportive experience for clients and families.

Requirements

  • Bachelor’s degree preferred; equivalent education and relevant experience considered
  • Minimum of 2 years of experience in residential treatment, facilities management, or behavioral healthcare preferred
  • Minimum of 1 year of experience working with youth and families
  • Strong organizational skills with exceptional attention to detail
  • Ability to manage multiple priorities in a fast-paced environment
  • Excellent verbal and written communication skills, particularly in emotionally sensitive situations
  • Proven ability to collaborate effectively with multidisciplinary clinical teams

Responsibilities

  • Coordinate with the Clinical Director, Admissions team, and clinical staff to manage the intake process from start to finish
  • Complete intake assessments and ensure all admissions documentation is accurate and complete
  • Welcome new clients and orient them to the treatment milieu
  • Conduct facility tours, phone assessments, and additional admissions-related tasks as needed
  • Communicate professionally and compassionately with clients, families, referral sources, and internal team members
  • Support outcomes research initiatives by gathering client feedback, reviews, and assessment data
  • Coordinate and ensure completion of all discharge procedures, including returning client belongings and finalizing documentation
  • Collaborate with clinical and admissions teams to ensure timely, organized, and supportive discharges
  • Support continuity of care through clear communication and documentation
  • Maintain accurate and timely clinical documentation and support chart audits as needed
  • Participate in treatment team meetings, staff meetings, and in-service trainings
  • Assist with onboarding and training for staff involved in intake and discharge processes
  • Demonstrate professionalism in appearance, communication, and conduct in alignment with agency values and philosophy
