About The Position

The Discharge Planner is a critical position of the Therapeutic Shelter’s functioning and organization. The Discharge Planner is responsible for assessing individual client need and will work to develop a plan that promotes long-term health and relapse prevention for male clients admitted to the Therapeutic Shelter. Responsible for facilitating referrals to aftercare programs and all administrative aspects of discharge planning. The Discharge Planner will work with a multi-disciplinary treatment team to obtain placement for clients currently in the behavioral health inpatient treatment setting. The Discharge Planner will also facilitate educational groups on topics related to obtaining basic needs, achieving educational and vocational goals, and developing continuing care plans. This position requires flexibility with location and skillset.

Requirements

  • Associates Degree with case management, discharge planning and/or client advocacy work required
  • Valid driver’s license, reliable transportation and valid auto insurance
  • CPR and first aid

Nice To Haves

  • Bachelor's degree from four-year college or university in Human Services or related field preferred.
  • Recovery Support Specialist Certificate can be considered with experience in similar role.
  • Experience working with individuals with substance use disorders, co-occurring disorders or severe mental illness is preferred.
  • Interest in working with a multi-disciplinary team is strongly encouraged.
  • Training preferred on the following topics: Treatment Criteria for Addictive, Substance-Related, and Co-Occurring Conditions (ASAM), Co-occurring disorders, including prevalence, signs/symptoms, assessment, treatment and the impact of one’s own attitudes/beliefs on the delivery of services and the use of naloxone in response to an opioid overdose.

Responsibilities

  • Responsible for obtaining aftercare placement for clients admitted to The Therapeutic Shelter. Including hospitalization, long term treatment centers, lower levels of care and recovery housing.
  • Ensuring all basic needs and treatment needs will be met after discharge from the program.
  • Act as the liaison between Wellmore, discharge placements and referral sources.
  • Develop and maintain positive working relationships with other community providers.
  • Actively engage and assist eligible clients in gaining public insurance, DSS application process and connection to entitlements such as Medicaid/Medicare.
  • Facilitate Psycho-educational groups on topics related to obtainment of resources, educational and vocational goal setting and achievement, relapse prevention planning and wellness planning.
  • Work cohesively with the Wellmore Adult Division’s Admissions Coordinator to ensure aftercare placement in Wellmore’s Outpatient programs and recovery house as appropriate. Including primary care and medication assisted treatment programs.
  • Responsible for completing referrals for clients to ensure medication needs are met; such as psychiatric evaluation and follow up, primary care and medication-assisted treatment.
  • Assist with daily program functions as needed, such as smoke breaks, supervising self-administration of medication and transportation.
  • Assist with coverage of co-workers’ vacations and other time out of the office.
  • Develop and maintain positive working relationships with management/leadership, coworkers, and clients utilizing a person centered and team approach.
  • Coordinate with treatment team as needed to ensure that clients and their needs are addressed as part of the holistic, whole-person, gender responsive, culturally sensitive integrated care approach.
  • Participate in supervision, meetings, agency committees and work groups as assigned by management.
  • May be required to participate in trainings (both on site and off site) as determined by the Director of Residential Services and/or Vice President of Adult Services.
  • Complete other duties as assigned.
