About The Position

The Intake & Discharge (I&D) Coordinator supports our Behavioral Health Outpatient programs by providing timely, client centered intake and initial support services. The I&D Coordinator is responsible for managing referrals from Santa Clara County Centralized Intake Line as well as other sources, completing intake or assigning for intake to ensure timely access to care. The I&D Coordinator also serves as a resource specialist, providing linkage and referral to appropriate services at intake or discharge. The I&D Coordinator carries a small caseload of clients who are preparing to discharge back to a “medication only” level of care, providing case management and linkage to ongoing supports in preparation for graduation from services.

Requirements

  • Bachelor's degree (B.A. or B.S.) from four-year college or university in a human services field or 2 years of related experience, or equivalent.
  • Bilingual communication skills (Spanish/English) are highly desired.
  • Ability to work well with a variety of diverse children, adolescents, and adults.
  • Knowledge of local community resources desired.
  • Understand and follow documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to document activities related to services provided.
  • Ability to communicate clearly and professionally with clients and families.
  • Ability to participate effectively in group discussions and meetings.
  • Must have a valid California driver’s license and access to a personal vehicle with current insurance that can be used for company business.
  • Personal cellular phone that can be used for company business.
  • Current CPR and First Aid Certification required (or complete within 30 days of hire).
  • Pass a background check and fingerprinting.
  • Pass a TB test.
  • Basic math skills to track hours, productivity, and expenses for reimbursement.
  • Ability to solve practical problems.
  • Ability to remain calm in escalated situations.
  • Intermediate computer skills required, including learning new software and applications.

Responsibilities

  • Administrative oversight and data entry related to referral management.
  • Make timely contact with referred individuals.
  • Complete intakes into services.
  • Assess immediate client needs and screen for risk factors.
  • Coordination and staffing collaboration with program assistants and supervisors for case assignment and triage.
  • Process and manage transfers into or out of agency programs.
  • Coordinate care and facilitate transfer of care to Primary Care Physician (PCP) or Managed Care Plan (MCP).
  • Lead group outreach for specific underserved groups (e.g., unhoused young adults, LGBTQ+ young adults) as needed.
  • Complete all required client documentation in the electronic health record following agency and county standards.
  • Meet expectations related to billable service hours (20-30%).
  • Coordinates and collaborates regarding supportive services, and client needs with interdisciplinary teams and provides linkage and referral as appropriate.
  • Maintain familiarity with available county resources and referral processes.
  • Meet the minimum for billable service time (20-30%).
  • Manage your own schedule to effectively balance client appointments and other tasks during your work hours.
  • Provide services in the community, including traveling to clients' homes, schools, or other locations.
  • Attend required meetings and training.
  • Participate in activities to improve quality and safety.
  • Keep the work area clean and report any maintenance or safety concerns immediately.
  • Regular attendance is essential for this job.
