Requirements

Strong communication, organization, and attention to detail.
Ability to work independently and manage multiple priorities.
Experience in intake, home health, medical office, scheduling, or care coordination preferred.
Comfortable working in electronic medical record systems.
A compassionate, patient-centered approach.
Dependable and committed to providing excellent service to patients, families, and referral partners.

Responsibilities

Process and finalize new patient referrals in our Homecare Homebase electronic medical record system.
Complete patient welcome calls and document coordination notes.
Follow up on pending and hold patients, including contacting hospitals and rehabilitation facilities for updates.
Complete pending and hold reports for assigned districts.
Work closely with the scheduling team to support a smooth transition into home health care.
Provide clerical support to the Nursing Supervisor.
Answer incoming calls, determine the appropriate response, and connect callers with the appropriate resource.
