Role Scope: The Intake Coordinator plays a key role in facilitating the intake process for prospective participants as they transition into Habitat Health's PACE program. This role coordinates intake activities, including home visits, assessments, and center tours, while partnering with prospective participants, caregivers, and cross-functional teams to ensure a seamless, participant-centered experience. Through proactive coordination, effective communication, and timely follow-through, the Intake Coordinator removes barriers, supports informed enrollment decisions, and contributes to a smooth transition into care.
