About The Position

The Intake Coordinator plays a critical role in ensuring a seamless, efficient intake process for patients requiring home health services. This position serves as the primary point of contact for patients, families, and healthcare providers, coordinating the collection and verification of necessary information to initiate care. The coordinator is responsible for managing referrals, scheduling initial assessments, and facilitating communication between clinical teams and external parties. By maintaining accurate records and adhering to compliance standards, the role supports the delivery of high-quality patient care. Ultimately, the Home Health Intake Coordinator contributes to patient satisfaction and operational efficiency by streamlining the intake workflow and addressing any barriers to timely service initiation.

Requirements

  • Minimum one year of experience in referral intake is preferred
  • Experience with third-party payers, Medicare, and Medicaid is highly desirable
  • Medical Field background preferred, including familiarity with medical terminology
  • Excellent computer and customer service skills
  • Valid Driver’s License
  • Valid Auto Insurance
  • HomeCare HomeBase experience preferred

Responsibilities

  • Receives and appropriately documents new referrals promptly.
  • Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications.
  • Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
  • Provides clear communication to patients, staff, other healthcare professionals, and referral sources.
  • Responds to Agency and patient needs in a professional and creative manner.
  • Demonstrates commitment, professional growth, and competency.
  • Participates in Agency-sponsored in-service training.
  • Participates in the Agency’s QAPI program.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
