About The Position

As an Intake and Eligibility Caseworker for the Office of Aging and Disability Services, you will respond to referrals from community members for Developmental Services Case Management eligibility for adults with intellectual disabilities and/or autism. You will meet with individuals, families, case managers and other stakeholders during the intake process to assess client needs while gathering information required for an eligibility determination. You will enter information in the OADS database system, Evergreen, for intake and eligibility purposes by required deadlines. You will complete outreach and education on the Intake and eligibility process with community members in school settings. You will also provide state case management for a small number of cases on a limited basis.

Requirements

  • Bachelors Degree from an accredited educational institution in Social Work/Social Welfare; OR a Bachelors Degree in a related social service/social welfare/social work area which includes at least 12 courses in behavioral science, social science, or social work
  • Must have or be eligible for conditional or full licensure as a Licensed Social Worker (LSW) as determined by the Maine State Board of Social Worker Licensure.
  • At least 1 year of experience working with incapacitated and dependent adults.
  • Must have or be eligible for conditional or full licensure at the Licensed Social Worker (LSW) level as issued by the Maine State Board of Social Worker Licensure.
  • Possession of a valid Class C Maine Motor Vehicle Operator's License.

Responsibilities

  • Respond to referrals from community members for Developmental Services Case Management eligibility for adults with intellectual disabilities and/or autism.
  • Meet with individuals, families, case managers and other stakeholders during the intake process to assess client needs while gathering information required for an eligibility determination.
  • Enter information in the OADS database system, Evergreen, for intake and eligibility purposes by required deadlines.
  • Complete outreach and education on the Intake and eligibility process with community members in school settings.
  • Provide state case management for a small number of cases on a limited basis.
  • Maintaining accurate referral reports with data and submitting them to your supervisor in a timely manner regarding referrals.
  • Gathering information in a complete and accurate manner to fully document all pertinent facts for eligibility determinations.
  • Meet with transition specialists within the department on individual cases for review.
  • Determining when to notify and consult with the Casework Supervisor and others.
  • Accurately assessing crisis situations and need for emergency intervention and notification of supervisory staff if such needs arise during intake.
  • Present to members of the public in group settings, transition fairs, and meetings the role of intake and eligibility.
