The Intake Coordinator has several supportive roles within the franchised pharmacy. This job summary describes various administrative and clerical roles that may be referred to by many job titles within the home infusion pharmacy.

Requirements

  • Proven knowledge and skill in utilizing MS Office suite of software and pharmacy applications.
  • Good interactive skills with employees, referral sources, supervisors, and consumers.
  • Must have a high school diploma or equivalent.
  • Must be 18 years of age.
  • Licensed Nurse.
  • Registered with the State Board of Nursing.

Responsibilities

  • Identifies potential home infusion patients by communicating with stakeholders within the facility (e.g., social work, discharge planning).
  • Explains insurance benefit verification and any estimated deductibles and copayments to the patient and/or family.
  • Completely and accurately fill in required forms during referral intake for proper billing and documentation of care.
  • Intake referrals and process through completion.
  • Uploads referral information to corporate.
  • Input necessary information into Vital Systems for benefits retrieval.
  • Coordinate with home health patient admission and/or labs required and send orders if needed.
  • Complete new patient paperwork for admission.
  • Update calendar for maintenance of delivery schedules and coordinate deliveries and infusions in infusion suites.
  • Document all correspondence in Caretend progress notes.
  • Establish and maintain charts to ACHC specifications.
  • Coordinate with home health concerning order changes, labs, patient updates, nursing notes, etc.
  • Conduct necessary patient follow-ups.
  • Discharge patients when therapy completed and enter into CareEnds.
  • Assist elsewhere in the office as needed.
  • Be willing to learn new things and take on new responsibilities.
  • Act as a backup to the Nurse Manager/liaison when needed.
