Intake Coordinator- Hospice
About The Position
Parker Jewish Institute's Community Health Services program is seeking an experienced professional to oversee the intake function for Parker's community programs, ensuring patients accepted to the program are appropriate and evaluated timely by home care and hospice staff. This role is key in providing both quality patient care and actively working to expand our services, as Parker's Hospice and Home Care programs continue to grow. Parker Jewish Institute's Hospice and Home Health Care offers the finest in home health services, and provides comfort, relief, and peace of mind to individuals with advanced or life-limiting illness and their families. We are an intimate and unique not-for-profit program that provides personalized care from an interdisciplinary team of healthcare professionals to ensure the comfort, enhance the quality of life, and preserve the dignity of our patients. Requirements
- 1-2 years of prior experience in a care coordinator, intake, admissions or community health role preferred.
- Experience in a Home Care, Hospice or Community Health environment preferred.
- Proficient in EMR software.
Nice To Haves
- Familiarity with Netsmart is a plus.
