Intake and Assessment Supervisor
About The Position
The Intake and Assessment Supervisor provides day-to-day leadership for the agency's Intake and Assessment Department, directly supervising Intake Specialists and Assessment Therapists and coordinating closely with the Manager to keep participants moving smoothly from first contact through, assessment, and program assignment. This role monitors referral and intake volume, holds the team accountable to timely and trauma-informed service, and steps in on complex or crisis situations that need additional judgment or support. The Supervisor works closely with all FYI+ programs to coordinate care across the agency, troubleshooting obstacles that stand between participants and the services they need. The Supervisor also provides direct, hands-on support for crisis walk-ins, stepping in alongside Intake Specialists to screen, de-escalate, and safety plan with participants who present in person in urgent need. The Supervisor is responsible for the quality, accuracy, and confidentiality of intake and assessment documentation, and for keeping Streamline EHR data current across the department. This is a new and evolving role within a department built to provide a responsive, informed, unbiased, affirming, and inclusive approach to a very diverse population, including individuals from LGBTQ+, marginalized, underrepresented, and BIPOC communities who have varying life experiences including substance use and abuse, homelessness, gender variance, traumatization, legal involvement, system involvement, domestic violence, mental health needs, teen pregnancy, varied abilities and learning needs, and poverty. This position may require in-person support in various organizational leadership-approved settings including homes, schools, offices, shared spaces, and community outreach locations. When participating in leadership-approved online activities, being on-camera and actively engaged is expected. Non-traditional hours may be required. This position may require working outside of standard business hours due to unexpected walk-ins or crisis support needs.
Requirements
- Bachelor's degree in Social Work, Counseling, Psychology, or a related human-service field required
- Minimum three (3) years of experience in behavioral health intake, triage, case management, or related direct service
- Knowledge of behavioral health conditions, crisis response, and community resource and referral networks
- Experience working with both adults and youth

- At least one (1) year of supervisory, lead, or team-coordination experience preferred

Responsibilities
- Provide direct supervision to Intake Specialists, including case consultation, referral support, etc.
- Manage personnel functions for the Intake team, including hiring, on-boarding, scheduling, coaching, and performance evaluations
- Hold regular individual and team supervision, including reflective and clinical case consultation, with direct reports
- Monitor daily referral queues, intake volume, and triage response and turnaround times; catch and fix delays before they affect participants
- Serve as an escalation point for urgent or crisis referrals, providing consultation and backup coverage for Intake Specialists
- Respond to and directly assist with crisis walk-ins, providing trauma-informed screening, de-escalation, and safety planning alongside or in place of Intake Specialists when needed
- Ensure walk-in services follow agency SOP's for crisis response, coverage, and documentation
- Review and submit Critical Incident Reports
- Coordinate with Assessment Therapists and the Clinical Manager on caseload assignment and scheduling for diagnostic assessments, so referrals move without unnecessary delay
- Partner closely with all agency programs to coordinate care and resolve obstacles standing between participants and the services they need
- Track and report on referral-to-intake and intake-to-assessment timelines for department and CCBHC reporting
- Ensure participant registrations, referrals, and scheduling in Streamline EHR are accurate, complete, and current
- Develop, maintain, and train staff on department SOP's, scripts, forms, and workflows
- Review documentation for accuracy, timeliness, and compliance with HIPAA and 42 CFR Part 2
- Conduct periodic chart audits and quality assurance reviews of intake and assessment documentation
- Represent the Intake and Assessment Department in leadership and interdisciplinary treatment team meetings
- Maintain availability for on-call or after-hours consultation as needed to support crisis walk-ins and urgent referrals
- Handle escalated participant concerns and staff conduct issues, documenting formally when required
- Coordinate warm handoffs and community referral partnerships alongside the Intake team
- Provide direct coverage of Intake Specialist duties as needed
- Participate in agency-wide quality improvement activities related to intake and assessment metrics
