About The Position

We are looking for a Inpatient Rounding RN to join our collaborative team at Memorial Health! This role involves using the nursing process to deliver patient care, facilitating multidisciplinary rounds on select patients, and serving as the central hub of clinical coordination. The Inpatient Rounding Nurse prepares for daily bedside rounds, reviews patient history and condition, collaborates with primary nursing staff and ancillary disciplines, provides timely updates to providers, coordinates patient care partner and/or family involvement, assists with communication of discharge goals and plans, reinforces patient education, and aligns prioritization for rounding efficiencies and discharge expedition. The nurse will coordinate communication regarding patient progress and plans related to discharge, document findings, and attend daily Grand Multidisciplinary Rounds. Additionally, the role includes performing nursing delegations, completing wound assessments and documentation, assisting with departmental staffing, maintaining competencies and mandatory education, and fulfilling license requirements. Requirements

  • Graduate of an accredited school of registered nursing with current registration in the State of Ohio.
  • Licensed as an RN in the State of Ohio.
  • Completes twenty-four (24) contact hours of approved CE during each two (2) year licensure renewal period consistent with requirements of Ohio Board of Nursing.
  • Maintains continuous certification in American Heart Association’s BLS.
  • Attains American Heart Association’s ACLS and PALS certification within six (6) months of employment and maintains continuous certification.
  • Obtains Behavioral Health & De-Escalation training (BHDT) within six (6) months of hire and maintains certification annually.

  • Wound staging can only be provided by providers and nurses with wound certification.

Responsibilities

  • Uses the nursing process of assessment, diagnosing, planning, implementation, and evaluation to deliver patient care.
  • Facilitates multidisciplinary rounds on select patients under the care of the Hospitalist team.
  • Serves as the central hub of clinical coordination for comprehensive multidisciplinary bedside rounds in tandem with the Hospitalist and PharmD.
  • Prepares for daily bedside rounds, reviews patient history and condition, and reviews pertinent social factors.
  • Collaborates with primary nursing staff and ancillary disciplines.
  • Provides timely status updates and pertinent information to providers and consultants as delegated.
  • Coordinates involvement of patient care partners and/or families.
  • Assists with communication of discharge goals and plans of care.
  • Reinforces patient education.
  • Aligns prioritization for rounding efficiencies and discharge expedition.
  • Coordinates centralized and ongoing communication surrounding patient’s progress and plans as it relates to discharge and/or changes in discharge prioritization and document accordingly.
  • Attends daily Grand Multidisciplinary Rounds with assigned provider and follows with an update to charge RN and primary RN team.
  • Performs nursing delegations in emergent and non-emergent situations as clinical needs present or by request from management.
  • Complete wound assessment and documentation on patients with wound consults in Inpatient department.
  • Document and communicate findings and intervention recommendations to requesting provider.
  • Assists with departmental staffing needs as assigned.
  • Maintains all departmental, nursing, and house-wide competencies.
  • Attends all mandatory education and in-services (i.e., team training, safety, infection control, etc.); completes mandatory health requirements.
  • Maintain necessary requirements to fulfill license requirements.

  • Demonstrates regular and predictable attendance.
  • Performs within the prescribed limits of the hospital’s and department’s Ethics and Compliance program and is responsible to detect, observe and report compliance variances to their immediate supervisor, or upward through the chain of command, the Compliance Officer, or the hospital hotline.
