About The Position

Come join our Inpatient Rehab team! The Inpatient Rehab Social Worker is responsible for planning, development, and follow-up of an appropriate plan of care for patients in need of extended care placement or home care following discharge from the inpatient rehab facility. This role also involves providing supportive psycho-social assistance for appropriate patients/families, as well as providing short-term counseling as needed and making appropriate referrals.

Requirements

• Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.
• MSW licensure by the South Carolina Board of Social Work Examiners.

Nice To Haves

• Certification in case management preferred.
