About The Position

This role involves reviewing and assessing medication orders to identify, prevent, and resolve drug-related problems, recommending formulary alternatives, and compounding and dispensing medications. The pharmacist will also provide drug information and education to medical staff and patients, communicate with clinicians to facilitate care, and oversee technical personnel. Additionally, the role includes assisting with academic functions as a preceptor for pharmacy students, potentially floating to cover pharmacy needs, and providing patient counseling.

Requirements

  • Pharmacist credentialed from the Indiana Board Of Pharmacy obtained within 3 Months (90 days) of hire date or job transfer date required.
  • Bachelor's or advanced degree in Pharmacy required.

Responsibilities

  • Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
  • Recommend formulary alternatives when appropriate.
  • Compound and dispense medications for accurate delivery to patients.
  • Provide drug information and education to medical staff and patients.
  • Communicate with physicians and other clinicians as necessary to facilitate care.
  • Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
  • Assist with academic functions such as serving as a preceptor for pharmacy students.
  • May float to provide coverage of pharmacy needs.
  • Provide patient counseling efficiently and effectively when required.
