Requirements

  • Doctor of Pharmacy or Pharm.D. degree in pharmacy (foreign equivalent degree accepted)
  • License to practice pharmacy in the state of Wisconsin

Responsibilities

  • Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes, including, but not limited to advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses for appropriate indications
  • Individualize medication regimes using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients
  • Prepare and/or oversee the preparation and dispensing of prescription medications to patients and making decisions regarding generic/therapeutic substitutions
  • Oversee and be responsible for acquisition, storage and distribution of pharmaceuticals throughout the organization
  • Provide additional support and vision when developing and implementing long-term pharmaceutical goals, the purchase of equipment, and the provision of pharmacy services
  • Maintain appropriate records as required by law or by the organization
  • Monitor and report on the use of pharmaceuticals within the institution; this includes, but is not limited to medication errors, adverse reactions, drug use, evaluations, and special requests via the pharmacy director
  • Investigate and take appropriate actions to resolve pharmaceutical issues throughout the hospital, nursing home, and clinics
  • Maintain knowledge of current Infection Control Issues and practice
  • Maintain technician supervision and coordination in the absence of the pharmacy manager
  • Adhere to regular and predictable attendance
  • Personally interact, engage with, and care for patients
  • Ensure patient safety
  • Serve as an information resource for affiliated and non-affiliated entities in an effort to provide comprehensive patient care
  • Contribute to the professional attitude and appearance of the pharmacy by consistently maintaining a patient-safety-focused environment
  • Perform other duties as necessary or as assigned
