Requirements

  • Bachelors Pharmacy or Doctorate Pharmacy-Preferred
  • RPH - Reg Pharmacist
  • Minimum 2 years in Pharmacy as a pharmacy technician, intern and/or pharmacist Preferred

Responsibilities

  • Provides clinical pharmaceutical care to patients.
  • Prevents, detects, and resolves medication-related problems and makes appropriate evidence-based, patient-centered medication therapy recommendations.
  • Participates as an effective member of interdisciplinary care team(s) and communicates follow-up with team.
  • Effectively verifies patient medication orders for appropriateness and accuracy.
  • Provides effective medication education to patients, families, team.
  • Enhances the quality of delivery of care.
  • Actively participates in the development and implementation of performance/quality improvement and patient safety initiatives, including medication event/adverse reaction reporting & follow-up, clinical guideline development, disease management, medication use evaluation/improvement, cost containment actions.
  • Represents pharmacy services on institutional/practice area committees, quality unit councils.
  • Conducts monthly unit inspections of assigned area.
  • Demonstrates proficiency in pharmacy distributive services.
  • Demonstrates quality, safety and efficiency in the distribution process.
  • Actively collaborates and demonstrate teamwork with pharmacy coworkers to ensure communication, excellent customer service, safe and effective drug distribution.
  • Provides oversight for pharmacy technicians.
  • Participates in opportunities to improve the quality, effectiveness, and/or efficiency in distributing medications.
  • Supports the education and professional development of the Pharmacy Department and other hospital disciplines (nursing/medical/etc.).
  • Precepts residents, pharmacy students, or pharmacy technicians as assigned.
  • Participates colloquially in the development of new programs, services, and practices, educational activities of the pharmacy department and medical team.
  • Engages in professional self-development.
  • Completes medical center/pharmacy competency requirements by deadlines.
  • Establishes regular meetings with manager.
  • Enhances professional and technical knowledge/competency by attending seminars, continuing education programs, department meetings, etc.
  • Demonstrate active membership/service/leadership in professional organizations
  • Performs other duties as assigned
  • Complies with all Wellstar Health System policies, standards of work, and code of conduct.
