About The Position

Reports to the Director of Clinical Services and works cooperatively with the Director and other team members to effectively implement the Hospice Plan of Care. Consistently exercises discretion and judgment in performance of duties to assure quality care.

Requirements

  • Current South Carolina license as a Registered Nurse.
  • Ability to assess and respond to the needs of Hospice House patients in a compassionate and caring way.
  • Possesses skills needed to lead a team and work as part of a team.
  • Skilled in Nurse Practice, able to cope with family emotional stress and tolerant of individual lifestyles.
  • Able to lift 50 pounds from knee to chest.
  • Professional positive attitude required.
  • Ability to embrace the mission and culture of the organization.

Responsibilities

  • May serve as a charge nurse and/or staff nurse.
  • Supervises the care being rendered by CNA's and ancillary staff.
  • If serving as charge nurse, supervises the documentation and care being rendered by the LPN staff and other RN staff.
  • Assist with calling for coverage (for call offs and as needed).
  • Assist supervisor with evaluating performance of co-workers as required.
  • Reports problems to the Director of Clinical Services in writing and details actions that have been taken related to the identified problem.
  • Assist with chart audits, chart reviews and quality assurance activities.
  • Assist in monitoring activities and documentation to ensure the Hospice House remains in compliance with all regulatory agencies.
  • Provides personal care and assistance to patients in the Hospice House such as bathing, feeding, bed making, ambulation, transferring and other patient oriented tasks.
  • Consistently and accurately collects patient data including physical, emotional and psychosocial issues and documents these findings in a timely manner.
  • Provides assigned treatment procedures to include but not limited to enemas, dressing changes, range of motion exercises, medication administration, etc., in accordance with agency policy and the patient's plan of care. Demonstrates knowledge of medication administration and/or researches unfamiliar drugs prior to administration.
  • Observes, documents and communicates patient status and related information on the appropriate forms. Teaches and demonstrates essential care techniques to family care givers as appropriate.
  • Ensures complete and accurate documentation / care meeting agency and regulatory guidelines.
  • Attends and participates in in-service programs, scheduled staff meetings, quality assurance activities, memorial services and other organization sponsored events.
  • Performs other related functions as assigned.
