Responsibilities include assessing and managing medication therapy, verifying and entering medication orders, distributing medications, monitoring therapy, consulting with patients and providers, documenting treatment plans, and providing patient education. Pharmacists also actively participate in quality improvement, research, and educational activities.

Requirements

  • Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.
  • Commitment to excellence, accuracy, attention to detail and teamwork required.
  • Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
  • Current pharmacist license by the Board of Pharmacy in the state(s) of the job's practice site, or eligible to obtain within a time frame compliant with the site’s applicable employee licensure policy.

Nice To Haves

  • Other advanced degrees (e.g., Master’s, Ph.D.) and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor preferred.
  • Completion of pharmacy residency and specialization (e.g., PGY-2 and respective BPS certification), if applicable, preferred.
  • Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations preferred.
  • Relevant applicable pharmacy experience working in a progressive and collaborative practice environment preferred.
  • Experience and/or interest in teaching and/or precepting preferred.

Responsibilities

  • Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients.

  • Medication therapy monitoring.
  • Consultation, documentation of medication therapy plans.

  • Participates in collaborative practice and/or protocol-driven practice as appropriate.
  • Active members in quality improvement, research activities, and education as described in the job specific competencies.
