About The Position

Sentara RMH Medical Center is now hiring an Inpatient Case Manager. This role is responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. The primary focus is on patient transition to the appropriate level of care while facilitating clinical appropriateness and Length of Stay (LOS). This position directly supports in-hospital patients identified as needing comprehensive case management. The Inpatient Case Manager promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. A comprehensive care plan is developed and/or implemented based on assessment and evaluation, including the physical, mental, and psychosocial needs of the inpatient/caregiver.

Requirements

  • Associates Degree - Nursing
  • Bachelors Degree – Nursing
  • Master’s Degree - Nursing
  • Registered Nurse License (Required)
  • 1 year of Acute Care Nursing experience
  • 1 year of Case Management experience
  • 1 year nursing experience

Responsibilities
