About The Position

Sentara Leigh Hospital, located in Norfolk, Virginia is hiring a full time RN Case Manager for our Emergency Department. This is a full-time position with 40 hours per week, consisting of five 8-hour shifts, Monday through Friday. The Case Manager will be assigned to the Emergency Department and requires a minimum of 12 months of nursing experience. This role involves providing care to patients and families throughout their inpatient stay, acting as a clinical leader, and developing the safest and most effective transition plans. The Case Manager will collaborate with nurses and social workers, utilize complex critical thinking skills, and promote effective utilization of health services. The primary focus is on patient transition to the appropriate level of care while facilitating clinical appropriateness and Length of Stay (LOS). The role directly supports hospital patients identified as needing comprehensive case management and develops comprehensive care plans addressing physical, mental, and psychosocial needs.

Requirements

Responsibilities

  • Provide care to the patient and family over the entire inpatient stay
  • Develop a relationship with the patient and family
  • Act as a clinical leader of the healthcare team
  • Drive decisions to assure the safest, most effective transition plan for patients and families
  • Work in a team of nurses and social workers, using each other's expertise
  • Help to drive hospital and organizational strategic initiatives in always improving patient care
  • Utilize complex critical thinking skills
  • Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.
  • Directly support those in hospital patients identified in need of comprehensive case management.
  • Promote effective utilization and monitoring of health services, collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
  • Develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.
