About The Position

Sentara Obici Hospital in Suffolk, VA, is seeking an experienced Inpatient Case Manager MSW to focus on complex cases. This role is responsible for providing medical social work services to patients in the hospital setting. The Inpatient Case Manager MSW assesses patients' social, cultural, environmental, and financial situations, as well as their disposition needs. They collaborate with healthcare team members and community organizations to identify needs and resources, and in consultation with the healthcare team, develop effective transition plans. This role also collaborates with the RN Case Manager to ensure a smooth patient flow throughout their hospital stay.

Requirements

  • Master's Level Degree in Social Work
  • Certification in Case Management (CCM, CMAC or ACM-SW) is required within one year of eligibility
  • BLS is required within 90 days of hire
  • 1 year of related acute care and/or healthcare experience required (Educational internship experience in an acute care facility can be considered OR Healthcare experience refers to the care delivered in one of the following settings: hospital or an acute care facility)

Responsibilities

  • Provision of medical social work services to patients receiving care in the hospital setting.
  • Assessment of the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient.
  • Collaboration with healthcare team members for the patient and community organizations to identify needs and resources in the hospital and community.
  • Development of effective transition plans in consultation with the healthcare team.
  • Collaboration with the RN Case Manager to ensure a smooth flow of patients through their hospital stay.
