About The Position

The Inpatient Case Manager, LVN is responsible for coordinating and facilitating appropriate patient care, repatriation to in-network hospitals, and discharge planning in collaboration with physicians, hospital staff, and external partners. The PHP Inpatient Case Manager follows patients admitted at PIH, Out of Network and Skilled Nursing facilities. This role ensures compliance with regulatory requirements, promotes efficient resource utilization, and supports high-quality patient outcomes. The Inpatient Case Manager serves as a liaison between multiple stakeholders to ensure continuity of care, timely discharge, and adherence to payer guidelines. Requirements

  • Understanding of inpatient case management principles, care coordination, and discharge planning
  • Ability to work in a fast-paced environment with strong multitasking and attention to detail
  • Strong communication, collaboration, and problem-solving skills
  • Computer literate, with ability to navigate multiple computer applications
