Inpatient Care Manager - Substance Use Disorders

In partnership with clients and/or families, this role works to facilitate mental health, and if applicable, intellectual disability/autism services, and substance use disorder recovery services. Requirements

  • Minimum Bachelor’s in the human services field with relevant course work required. (ID Support, Early Intervention, Substance Use Disorders)
  • High School Diploma or GED with 12 semester credit hours in the human services field. (Victory for Veterans program only).
  • Minimum Bachelor’s Degree in the human services field with relevant coursework required. or a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers. (Mental Health Case Management only)
  • Must possess and maintain a valid driver’s license along with a driving record in compliance with St. Luke’s Penn Foundation policy.

Responsibilities

  • Provide supportive services including outreach, monitoring, assessment, services planning, crisis intervention, support network building, mentoring, coaching, initial screenings, locate, coordinate and monitor needed services; in compliance with departmental policies and procedures.
  • Complete referrals and follow-up to all behavioral health supports identified; in compliance with departmental policies and procedures.
  • Ensure coordination and collaboration with all involved staff and applicable community members.
  • Utilize de-escalation and crisis diversion skills and strategies to support participants who are in crisis.
  • Maintain productively standards and complete all required documentation according to departmental guidelines.
  • When applicable, participate in rotating on-call as required.
