About The Position

The Charles Lea Center is seeking a full-time Innovation Coordinator to support people in Dickson and surrounding counties in Tennessee. This role focuses on helping individuals live more effective and meaningful lives by securing non-traditional services that combine staff support and enabling technologies. The Innovation Coordinator will work under the guidance of the Regional Manager of Innovative Services, focusing on assessment, person-centered planning, linking people to services, training, and advocacy. The position involves developing and implementing a new service model in Tennessee for young adults, emphasizing person-centered, employment-first, and technology-first approaches. The Charles Lea Center is a CQL accredited organization with services in South Carolina, North Carolina, and now Tennessee.

Requirements

  • At least 18 years of age
  • Must be able to effectively read, write, and communicate verbally in English
  • Ability to pass a background check upon hire
  • Valid driver's license
  • Vehicle Insurance
  • A bachelor’s degree from an accredited college or university in a human services field; or
  • A bachelor’s degree from an accredited college or university in a non-related field and one year of relevant experience; or
  • Associate degree and 2 years of relevant experience; or
  • High School Diploma or General Equivalent with 4 years of relevant case management experience providing or coordinating services to people with disabilities and special needs and/or with case management services.
  • Relevant experience means experience working directly with persons with intellectual, developmental, or other types of disabilities or mental illness.
  • Able to work independently as well as with a team
  • Exhibit a good work ethic and professionalism
  • Genuinely like to help people and have the drive to build your career on doing good

Responsibilities

  • Support people to live more effective and meaningful lives by working with them to secure nontraditional services that encompass a combination of staff supports and enabling technologies.
  • Provide services and supports focused on achieving goals for the people we support.
  • Provide technical assistance to Community Providers implementing services for people referred.
  • Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian.
  • Upon successfully transitioning to community services conduct 30, 60, 90 day follow up to assure satisfaction with services and those desired outcomes are being achieved.
  • Follow the standards and regulations required by Tennessee Department Disability and Aging (DDA).
