About The Position

The Charles Lea Center is seeking an Innovation Coordinator to help develop and implement a new service in Tennessee designed to assist young adults in their communities. This role utilizes a unique approach centered on person-centered, employment-first, and technology-first principles. The Innovation Coordinator will be grounded in person-centered thinking and foster a creative, forward-thinking approach to service delivery in Tennessee. The Innovation Coordinator works under the guidance of the Regional Manager of Innovative Services, primarily supporting individuals to live more effective and meaningful lives by securing nontraditional services that combine staff supports and enabling technologies. This includes residential, employment, and community supports, with key areas involving assessment, person-centered/MAPs planning, linking people to services, training, and advocacy.

Requirements

  • At least 18 years of age
  • Must be able to effectively read, write, and communicate verbally in English
  • Ability to pass a background check upon hire
  • Valid driver's license
  • Vehicle Insurance
  • A bachelor’s degree from an accredited college or university in a human services field; or A bachelor’s degree from an accredited college or university in a non-related field and one year of relevant experience; or Associate degree and 2 years of relevant experience; or High School Diploma or General Equivalent with 4 years of relevant case management experience providing or coordinating services to people with disabilities and special needs and/or with case management services.
  • Relevant experience means experience working directly with persons with intellectual, developmental, or other types of disabilities or mental illness.

Responsibilities

  • Provide services and supports focused on achieving goals for the people we support.
  • Provide technical assistance to Community Providers implementing services for people referred.
  • Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian.
  • Upon successfully transitioning to community services conduct 30, 60, 90 day follow up to assure satisfaction with services and those desired outcomes are being achieved.
  • Follow the standards and regulations required by Tennessee Department Disability and Aging (DDA).
