About The Position

This position involves providing specialized home infusion therapy services. The role requires assessing patients, developing individualized care plans, and educating patients and caregivers. Collaboration with healthcare providers, pharmacies, and interdisciplinary teams is essential for ensuring safe and effective care. The Infusion RN will monitor patients for reactions and changes in their condition, maintain accurate documentation, and support quality improvement initiatives. Home infusion nursing offers a flexible schedule, a high level of autonomy, and the opportunity to build long-term relationships with patients. Nurses can expect to be trained on and administer specialty biologics, IVIG, and manage rare diseases, with opportunities for certification. Venture Forthe specifically provides a pathway for Infusion RNs to obtain a company-sponsored Immunoglobulin Certified Nurse (IgCN) certification, allowing them to become experts in administering and monitoring patients receiving specialty medications.

Requirements

  • Current New York State Registered Nurse (RN) license.
  • Graduate of an accredited nursing program.
  • Strong communication, organizational, and critical thinking skills.
  • Ability to work independently in varied home environments.

Nice To Haves

  • At least 1 year of acute care nursing experience preferred.
  • Infusion therapy experience strongly preferred, including IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, and hydration therapy.
  • Home care experience preferred but not required.

Responsibilities

  • Provide specialized home infusion therapy services, including IV initiation, administration, monitoring, dressing changes, blood draws, and pump programming.
  • Assess patients and develop individualized infusion plans of care.
  • Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
  • Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
  • Monitor patients for infusion reactions, side effects, and clinical changes.
  • Maintain accurate and timely clinical documentation.
  • Support quality improvement initiatives and ongoing competency programs.
  • Assist with coordination care between physicians and specialty pharmacies.
