About The Position

Soleo Health is hiring a full-time Infusion Nurse to serve patients in Denver, Colorado, and the immediate surrounding area. The nurse provides primary care for assigned patients receiving infusion therapy or related nursing services in their homes or alternate care settings. This includes assessment, care planning, evaluation, patient and care-partner education, and coordination with the clinical team in accordance with care matrices and best practices. The position is scheduled for 40 hours per week, generally Monday through Friday, 8:30 a.m. to 5 p.m. Flexibility is required for occasional evenings and weekends, participation in an on-call rotation, and responding to emergencies during non-standard hours when needed. The nurse must be comfortable administering infusions in patients’ homes and able to drive up to one hour to visit patients, or up to two hours as needed. Mileage reimbursement and paid drive time are provided.

Requirements

Current RN license in good standing from the applicable state Board of Nursing; Colorado licensure is required.
Minimum of three years of nursing experience; an infusion therapy background is preferred.
Knowledge and experience with IV access devices and infusion administration, including peripheral IVs, central lines, PICCs, port access, and infusion-pump troubleshooting.
Ability to administer infusions in patients’ homes.
Ability to drive up to one hour to patient homes, and up to two hours as needed.
Valid state-issued driver’s license without significant restrictions.
Proficiency with Microsoft Outlook and Microsoft Office Suite.
Availability for the nurse on-call rotation and prompt response to emergencies, including nights, weekends, and holidays as required.
Flexibility to work occasional evenings and weekends as needed.

Nice To Haves

Bachelor’s degree in nursing (BSN) from an accredited institution.
Advanced specialty certification, such as CRNI, ONS, or IgRN.
Experience in infusion therapy.
Experience in ICU and/or cardiac care.
Familiarity with AlayaCare, Insight, CPR+, Concur, and Paylocity.

Responsibilities

Provide direct care to assigned patients and maintain communication with pharmacists, prescribers, dietitians, care partners, home patient representatives, and relevant community resources.
Evaluate and educate patients and care partners on safe administration of home infusion therapy, providing additional instruction as indicated.
Assess the home environment and identify interventions needed to make it safe and appropriate for prescribed care.
Develop plans of care with other team members and evaluate patients’ responses to therapy during each encounter.
Manage the schedule through the company-approved platform and promptly communicate scheduling changes to relevant team members.
Coordinate patient care through the approved company platform.
Participate in the nurse on-call rotation to provide continuous nursing support.
Document patient encounters, communications, and care activities at the time of service in accordance with company policy and regulatory requirements.
Maintain knowledge of Soleo therapies, protocols, and clinical standards.
Submit plans of treatment and other required clinical documents on time, in accordance with policy and state guidelines.
Work with relevant departments to ensure required patient documents, including consents and plans of treatment, are completed and returned to the branch office within the required timeframe.
Provide administrative oversight by monitoring workflows, ensuring policy compliance, and managing resources efficiently.
Respond promptly to emergencies, including during non-standard hours as required.
Perform other duties as assigned.
