Requirements

  • Current New York State Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • At least 1 year experience with infusion therapy which may include IV Therapy, PICC Lines, Central Line Care, Biologics, TPN, IV Antibiotics, and/or Hydration Therapy
  • Strong communication, organizational, and critical thinking skills
  • Ability to work independently in varied home settings
  • Must be authorized to work in the United States without current or future sponsorship of any kind.

Nice To Haves

  • At least 1 year of Home Care experience
  • At least 1 year of hospital experience

Responsibilities

  • Provide specialized home infusion therapy services, including IV initiation, monitoring, dressing changes, blood draws, and pump programming.
  • Assess patients and develop individualized infusion plans of care.
  • Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
  • Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
  • Monitor patients for infusion reactions, side effects, and clinical changes.
  • Maintain accurate and timely clinical documentation.
  • Support quality improvement initiatives and ongoing competency programs.
  • Assist with training and supervision of paraprofessional and nursing staff.
