About The Position

Soleo Health is seeking a per diem Infusion Nurse to support patients in Auburn, Alabama, and surrounding areas. The nurse provides primary patient care for assigned patients receiving infusion therapy and related nursing services in the home or an alternate-site setting. Care includes assessment, care planning, evaluation, and patient and caregiver education in accordance with care matrices and best practices. The nurse coordinates care with the appropriate clinical team and community resources, evaluates the safety of the care environment, develops and reviews plans of care, manages scheduling and documentation, and ensures required treatment plans and clinical documents are submitted and returned according to company policies and state guidelines. The role requires living within 25 minutes of Auburn and being able to drive a personal, properly registered and insured vehicle for trips of up to two hours each way as needed. The position is per diem, approximately 8–24 hours per week on a variable schedule. The schedule requires availability Monday through Friday, 8:30 a.m. to 5 p.m. The posting also requires availability to respond to emergencies, including during non-standard hours, nights, weekends, and holidays, as needed.

Requirements

Current single-state RN license in good standing for Alabama.
RN degree; a Bachelor of Science in Nursing from an accredited institution is preferred.
Home Health or Home Infusion experience is required.
At least 3 years of nursing experience; infusion therapy background is preferred.
Experience using IV access devices, including IV lines and accessing ports, and administering infusion medications.
Valid state-issued driver’s license with no significant restrictions.
Ability to drive a properly registered and insured personal vehicle up to two hours one way as needed.
Ability to work Monday through Friday, 8:30 a.m. to 5 p.m., on a variable per diem schedule of 8–24 hours per week.
Must live within 25 minutes of the Auburn, Alabama area.
Proficiency with Microsoft Outlook and Microsoft Office Suite.
Availability to respond to emergencies, including nights, weekends, and holidays as required.

Nice To Haves

Bachelor of Science in Nursing from an accredited institution.
Advanced certification in a specialty area, such as CRNI, ONS, or IgRN.
Infusion therapy background.
Experience in ICU and/or Cardiac Care.
Experience with AlayaCare, CPR+, Insight, Concur, or Paylocity.

Responsibilities

Provide direct care to assigned patients and maintain communication with pharmacists, prescribers, dietitians, patient representatives or care partners, and appropriate community resource staff.
Evaluate and educate patients and care partners on safe administration of home infusion therapy, providing additional instruction as indicated.
Assess the home environment and identify interventions needed to make it safe and appropriate for prescribed care.
Develop plans of care with other team members and evaluate patients’ responses to therapy at each encounter.
Manage schedules using the company-approved platform and promptly communicate scheduling changes to appropriate team members.
Coordinate patient care through the approved company platform.
Document patient encounters, communications, and care activities at the time of service in accordance with company policy and regulatory requirements.
Maintain knowledge of Soleo therapies, protocols, and clinical standards.
Submit plans of treatment and required clinical documents on time in accordance with policy and state guidelines.
Work with appropriate departments to ensure required patient documents, including consents and plans of treatment, are completed and returned to the branch office within an acceptable timeframe.
Provide administrative oversight, including monitoring workflows, ensuring policy compliance, and managing resources efficiently.
Respond promptly to emergencies, including outside standard hours when required.
