About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. POSITION SUMMARY The Registered Nurse is a licensed health care professional with advanced medical knowledge who administers skilled nursing care to patients requiring intermittent visits. She/he provides complex care to the patient and coordinates necessary support services. These nursing services are provided to patients in accordance with a physician’s plan of care. Registered Nurses who serve as primary nurses utilize a case manager approach in assessing, implementing, coordinating and evaluating client care. Additionally, the Registered Nurse manages other members of the nursing team. We are seeking a Registered Nurse that is looking to specialize in intravenous (IV) therapy.

Requirements

Graduate of state-approved school with course in pharmacology
Current license in the State of Pennsylvania
Minimum of two [2] years experience in patient care
Reliable, independent means of transportation to make home visits.
Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.
Previous experience with IV therapy
Experience with starting IV's, accessing ports, monitoring IV's in a patient's home, titrating meds for IVIG, and remicade.

Responsibilities

Administers skilled nursing care to patients requiring intermittent visits.
Provides complex care to the patient and coordinates necessary support services.
Provides nursing services in accordance with a physician’s plan of care.
Utilizes a case manager approach in assessing, implementing, coordinating and evaluating client care.
Manages other members of the nursing team.
Specializes in intravenous (IV) therapy.
Starts IV's, accesses ports, monitors IV's in a patient's home, titrates meds for IVIG, and remicade.
