About The Position

Under the direction of the Enrollment Manager and Chief Nursing Officer, this position is responsible for coordinating participant enrollment and intake services for Total Senior Care, Inc. It also serves as a liaison for HomeCare & Hospice with the professional medical community to build awareness of services and identify growth opportunities.

Requirements

  • Two years related work experience.
  • Computer literacy, including familiarity with Microsoft Office Suite and telecommunication systems.
  • Effective communication skills, including public speaking and written communication.
  • Acceptable driver’s license and automobile available for use with current insurance.
  • Availability by phone.

Nice To Haves

  • Associate degree preferred from an accredited college and/or institution in human services, communications, or similar field.

Responsibilities

  • Provides program information and follow-up to inquiries and referral sources.
  • Conducts facility tours and presentations of program benefits and services.
  • Identifies and educates healthcare professionals about services, developing and providing relevant marketing presentations.
  • Recommends new or revised marketing materials.
  • Assists with intake and enrollment processes, ensuring compliance with CMS PACE and NYS guidelines.
  • Complies with all marketing guidelines and prohibited marketing practices.
  • Assists prospective participants, patients, and caregivers with applications and documentation for financial eligibility.
  • Completes timely and accurate intake and enrollment documentation and maintains records.
  • Serves as a liaison to referral sources to foster relationships and support enrollment development.
  • Participates in quality improvement programs related to enrollment and intake.
  • Assists with monthly reports tracking educational visits and referrals.
  • Treats all people with dignity and respect, following privacy and participant rights policies.
  • Adheres to and reflects organizational values.
  • Serves on agency committees as assigned.
  • Reports wrongdoing or violations of policies, laws, or regulations.
  • Completes all mandatory inservice education programs.
