About The Position

Conduent is seeking an Information and Referral Specialist to support individuals by connecting them with essential services and resources. This role involves completing assessments, managing calls, and providing accurate referrals to community services. The position requires a Bachelor's degree in a human services field and at least one year of experience in social or human services, ideally with elderly or disabled adults. Familiarity with various government agencies and services related to aging, health, and developmental disabilities is important. The role also requires strong communication, organizational, and confidentiality skills, with proficiency in Microsoft Office.

Requirements

Bachelor’s degree in the human services field.
At least 1 year in social or human services, ideally with elderly or disabled adults.
Familiarity with Office of Aging and Adult Services (OAAS), Bureau of Health Services Financing (BHSF), Department of Health and Hospitals (DHH), Medicaid, Office for Citizens with Developmental Disabilities (OCDD), and home/community-based waiver services.
Understanding of medical and developmental disability terminology.
Ability to maintain strict confidentiality, work independently, and remain composed under pressure.
Proficiency in Microsoft Office (MS Office).

Nice To Haves

Additional experience in social/human services with elderly or disabled adults.
In-depth knowledge of OAAS, Medicaid programs, and related services.

Responsibilities

Complete Level of Care Eligibility Tool (LOCET) assessments and other required evaluations.
Manage inbound and outbound calls with professionalism and empathy.
Provide accurate referrals to non-Office of Aging and Adult Services (OAAS) community services.
Assess requestors’ needs and research available resources to meet those needs.
