About The Position

The Infectious Disease Program Manager is responsible for working directly with the Infectious Disease team to deliver comprehensive care to patients living with HIV, Hep C, TB, and other infectious diseases. This Manager, along with the Infectious Disease provider, leads the clinical team in the standard of HIV care. The Manager is responsible for personnel management, including hiring, supervision, evaluations, and terminations. They provide supervision of STI Clinic Staff and HIV Medical Case Managers, oversee day-to-day operations, and monitor caseloads, charts, and completion rates of Individual Service Plans and other Medical Care Coordination activities. The Manager also completes DPH reports monthly and annually, maintains the State and Federal CAREWare database, reports on grants, organizes team and patient meetings, handles coordination of procedures, delivery of specimens, assists providers, works with the ID nurse on high-risk patient care, handles triages, phone calls, orders medication, and directs MAs and Medical Case Managers. This individual oversees the STI walk-in clinic and HIV team workflows and collaborates with the Pediatric, OB/GYN, and HRC departments on infectious disease cases. Additionally, they maintain a professional and patient-centered environment, ensuring efficient patient flow and advocating for patients and their families.

Requirements

  • Bachelor of Science Degree in Nursing, Social Work, Public Health or related field preferred.
  • Knowledge of Infectious Diseases and standards of care.
  • Advanced Computer skills including Excel, Word, Teams, and other Microsoft apps.
  • Knowledge of DPH Reporting requirements and software.
  • Ability to complete monthly reporting to funders on progress toward Work Plan goals.

Responsibilities

  • Administrative and fiscal oversight of HIV and STI Clinic, Medical Case Management Services and Outreach activities.
  • Personnel management including hiring, performance evaluations, disciplining, and other activities.
  • Assist in the identification of funding sources, preparation of grant applications.
  • Grants management ensure that all grant fiscal, administrative, data collection, and reporting requirements are met.
  • Maintain program expenditures within budget parameters.
  • In collaboration with COO, Director of Nursing and Director of Substance Use Services, develops annual plan for community involvement and outreach.
  • Create and strengthen relationships with community partners, interfaith organizations, and government and state agencies.
  • Work within a management structure to collaborate with other Supervisors and Program Managers.
  • Other duties as assigned.
