About The Position

Abstracts, collects and analyzes data on healthcare events to reduce the incidence of such events and to protect patients, staff and visitors from organism/infections transmission or other quality and safety concerns. Informs clinical personnel of pertinent data on adverse healthcare associated events and quality of care outcomes. Performs analytic activities on assigned divisions recording demographic /clinical risk information as well as information on adverse outcomes including defined healthcare associated events to ensure safe quality care to all patients. Visits assigned patient, inpatient or ambulatory care areas at designated intervals for the purpose of collecting pertinent information concerning patients with community and hospital acquired infections and other adverse healthcare associated events as determined by pre-defined surveillance criteria. Plans work schedule to complete assigned reviews in a timely fashion as observed by supervisors. Accurately uses electronic systems to record clinical information and adverse outcome data. This includes data entry, data correction and regular updating of records as necessary.

Requirements

Bachelor's Degree in clinically related field
Healthcare experience preferred.
Preference given to persons with bedside experience.
Special interest in epidemiology, infection prevention and/or clinical quality and safety; highly motivated, tactful in dealing with others; ability to handle stressful situations; willingness to learn and apply knowledge appropriately.
Ability conducting telephone-based or in-person interview, data collection and data entry are preferred.
Experience with health education, social services, or other public health services preferred.

The position will require daily use of a computer, telephone/cell phone, and access to a secure internet connection.
Involves viewing a computer screen approx. 50-80 percent of the time.
The position requires both onsite and telework activities, while complying with all requirements for client confidentiality, records, and data security.

Responsibilities

Performs analytic activities on assigned divisions recording demographic/clinical risk information as well as information on adverse outcomes including defined healthcare associated events to ensure safe quality care to all patients.
Visits assigned patient, inpatient or ambulatory care areas at designated intervals for the purpose of collecting pertinent information concerning patients with community and hospital acquired infections and other adverse healthcare associated events as determined by predefined surveillance criteria.
Plans work schedule to complete assigned reviews in a timely fashion as observed by supervisors.
Accurately uses electronic systems to record clinical information and adverse outcome data. This includes data entry, data correction and regular updating of records as necessary.
Completes infection related data abstraction and updates information on a timely basis.
Alerts clinical staff to potential quality of care concerns, infection prevention problems and isolation needs. (Example: Reviews all positive laboratory findings daily to identify antibiotic resistant microorganisms, epidemiologically significant organisms and healthcare acquired conditions when assigned to infection prevention analytic work).
Reports complex problems to clinical personnel for further follow-up.
Assists other Infection Prevention or Quality and Safety staff in investigation such as cluster and outbreak investigations, chart reviews, audits and other occurrences as requested.
Keeps accurate records on all problems investigated. Documents investigation and problem resolution and enters information intoappropriate systems.
Demonstrates knowledge of appropriate resources for obtaining supplemental information on epidemiology and adverse hospital events.
Collects information on diseases and conditions reportable to the Department of Public Health and/or other regulatory/accrediting bodies. Completes reports, mails and/or fax within the required time frames
Seeks appropriate assistance and resources to resolve problems.
Demonstrates the ability to recognize patterns, trends and/or adverse outcomes, such as trends of outbreak/cluster investigation analysis which may require prompt preventive action.
Manages the planning, abstraction. surveillance and analytic activities as assigned. Keeps clinical personnel informed of the status activities and maintain scheduled deadlines. Notify clinical personnel and manager prior to deadline, when delays have occurred.
Participates in selected improvement projects as identified by Infection Prevention Manager by providing surveillance, analytics and other support as requested.
Performs various departmental duties as assigned by supervisor to enhance the efficient operation of the department.
In the absence of other support staff, provides work coverage and follows-up on problems requiring immediate attention.
Actively participates in huddles and departmental meetings and contributes pertinent information without being prompted.
Anticipates needs of coworkers and and provides active assistance.
Recognizes responsibility for continuing education needs to maintain accurate and current knowledge about infection prevention, patient safety and quality improvement.
Maintains competency by reading relevant infection prevention, safety and quality improvement and infection prevention articles. Shares information with department colleagues.
Actively participates in departmental education sessions. Consistently strives to maintain current knowledge base of methods, techniques and skills required for monitoring, surveillance and other analytic activities to ensure efficiency and safe quality patient care.
Engage in and complete self-directed or Manager designated professional development during each measurement period.
Communicates information to others in a clear and concise manner so that the message can be fully understood.
Responds promptly to requests for consultation on infection prevention matters. Maintains open and frequent communication with leaders and staff on patient care unit.
Shares pertinent information and keeps hospital personnel abreast of changes in infection prevention, quality and safety protocols to ensure safe quality patient care.
Communicates pertinent information regarding findings during daily surveillance activities to infection prevention staff members and Manager in a timely manner.
