About The Position

Individualized Home Supports (IHS) provide essential assistance to individuals with disabilities, empowering them to live as independently as possible in their own homes. IHS focuses on fostering independence, building daily living skills, and improving overall quality of life. By offering flexible, person-centered support, these services help individuals thrive in their preferred living environment and maintain greater autonomy in their day-to-day lives. This position provides a personalized blend of one-to-one in-home and in-community services that focus on the support and skill development a person needs to maintain and/or increase their independence and teach individual life skills. Services are customized to each person’s unique needs and may be delivered in-person or remotely to enhance comfort, safety, and accessibility.

Requirements

  • Proficient with Microsoft Office software (Outlook, Word, etc.)
  • Case management – ability learn software system and become proficient
  • Current, valid MN driver’s license
  • A driving record that demonstrates safe, legal driving habits
  • Compliance with Rule 11 and MVR background check required?

Nice To Haves

  • Previous experience practicing person-centered techniques with people with disabilities
  • Degree in human service field
  • Certification in medication management

Responsibilities

  • Support individuals in building decision making skills and making informed choices in all areas of daily life.
  • Assist with day-to-day activities such as personal care, meal preparation, transportation, housekeeping, and laundry.
  • Help maintain a clean, safe, and comfortable living environment that supports individuals in reaching their personal goals.
  • Provide skill development in areas including, but not limited to, community participation, health and wellness, safety skills, household management, and adaptive skills such as communication and social interaction.
  • Offer education and opportunities to build problem-solving skills related to safety, social interactions, and self-advocacy.
  • Dispense and document medication administration - pass medications and perform specific medicinal treatments/cares according to individual need.
  • Encourage positive behavior, provide appropriate supervision, and follow each person’s behavior support plan when applicable.
  • Be knowledgeable about crisis prevention, intervention and resolution techniques and match such techniques to circumstances and individuals. Identify the crisis, defuse the situation, evaluate and determine an intervention strategy and contacts necessary supports. Continue to monitor crisis situations, discussing the incident with authorized staff and person(s) served, adjusting supports and the environment, and complying with regulations for reporting. Use appropriate first aid/safety procedures when responding to emergencies.
  • Attend team meetings, attend check-ins with direct supervisors, and complete all required annual training courses on time, using Rise’s online learning management system.
  • Document service plan outcomes and write daily progress reports.
  • Oversee responsibilities of maintaining a caseload of 20 or more individuals.
  • Document service start and end time in order to provide accurate and timely billing.
