About The Position

Individualized Home Support Specialist (IHS) workers are ambitious, talented, kind, creative, and innovative. They are able to adapt to the changing needs of individuals who have mental health conditions or disabilities and chronic illnesses. In a caregiving way, they assist individuals develop, and encourage them. cope and adapt to their daily life toward quality living as they desire, create self-sufficiency and navigate life in their community. Through encouragement, IHS Specialists are able to empower their clients to improve their quality of life by teaching them skills and enriching their self-esteem. Clients served to develop the autonomy to make their own decisions and manage the way they live.

Requirements

  • Pass a Minnesota Department of Human Services Background Study.
  • Have A Minnesota Driver’s License.
  • Have Full Coverage Insurance.
  • Have a Reliable Vehicle.
  • Willingness to transport clients in your vehicle (Clients may have Walkers, Wheelchairs).
  • Able to document through Case Notes, expediently, capturing details of IHS Scheduled visits with clients weekly.
  • Be a team player, attend team meetings and offer ideas and resources.
  • Maintain confidentiality with professionals utilizing releases of information when necessary, adhering to all HIPPA regulations.
  • Be organized, ethical and trustworthy.
  • Technologically, adaptable as mileage, timecards and navigation are all done by apps.
  • Complete all training required by State of Minnesota 245D.

Responsibilities

  • Assist clients in developing skills to cope and adapt to daily life towards quality living.
  • Encourage clients to create self-sufficiency and navigate life in their community.
  • Empower clients to improve their quality of life by teaching them skills and enriching their self-esteem.
  • Assist clients in developing autonomy to make their own decisions and manage their lives.
  • Assist clients with Chores (home, laundry).
  • Assist with Mail/Paperwork organization.
  • Assist with Business needs (Bills, checkbook).
  • Accompany clients to Doctor/Dentist appointments.
  • Connect clients with Community Resources (Recreational/Housing/Employment/Community Support).
  • Assist with Meal Preparation and Grocery Shopping.
  • Document through Case Notes, expediently, capturing details of IHS Scheduled visits with clients weekly.
  • Be a team player, attend team meetings and offer ideas and resources.
  • Maintain confidentiality with professionals utilizing releases of information when necessary, adhering to all HIPPA regulations.
  • Complete all training required by State of Minnesota 245D.
