As an employee of the Fairfield County Board of Developmental Disabilities (FCBDD), the job incumbent shall comply with all Board policies at all times, and shall demonstrate respect for, support dignity of, and observe the rights of all individuals served by the agency. This role involves performing individualized service and support administration functions for designated individuals, assisting them in identifying choices, preferences, and strengths through a self-determination planning process. The coordinator collaborates with individuals, their teams, and providers to develop person-centered plans, behavioral support strategies, and manage individual budgets. This position requires maintaining documentation, monitoring services, responding to emergencies, developing community resources, and assisting with waiver funding processes. Additionally, the role involves communication, training, and other duties as directed.
