About The Position

This role involves assessing individuals to determine eligibility for home and community-based services that promote greater independence and wellbeing. This is a field-based remote position which will primarily allow you to work from home. Travel is required throughout Post Falls and the greater Idaho Panhandle region (up to 25%). This important work is accomplished through a collaborative partnership between the Idaho Department of Health and Welfare and Liberty Healthcare – an organization that is nationally recognized for supporting people who are living with disabilities. Requirements

• Professional or personal experience supporting people who are living with disabilities
• Outstanding verbal and written communication skills
• Ability to meet goals with high degree of autonomy
• Bachelor’s degree
• Relevant work experience
• Experience supporting people who are living with autism or developmental disabilities

Nice To Haves

• Worked as a Direct Support Professional, Case Manager, QIDP, Social Worker, Service Coordinator, Client Advocate, or Caseworker.

Responsibilities

• Leading virtual and in-person meetings with children and adults with developmental disabilities and autism
• Speaking with family members, guardians, and caregivers
• Administering structured assessments
• Completing reports
